Do you accept my insurance? 

We are in-network with Select Health, EMI, DMBA, PEHP in the state of Utah. We are in-network with DMBA in the state of Idaho. 

What is a superbill? 

A superbill is a detailed receipt for services that you can submit to an out-of-network insurance company for potential reimbursement. We provide these upon request. We recommend contacting your insurance provider for more information about your plan. 

How are benefit payments handled? 

If you are using your health insurance for therapy, we will submit the claim to your insurance, wait for the Explanation of Benefits and then use the card on file to pay your required portion.  

Why do I need a card on file? 

We require that all clients have an active credit card saved to their chart. If the card is no longer valid, you risk losing your therapy slot. You can log in to your Client Portal to add updated card information. Our office can also assist your if needed. Late Cancellation and No-Show fees will be charged to card on file. Those are not payable by insurance or other third-parties. 

What is your cancellation/no-show policy? 

There is a $75 fee for late cancellations (less than 24 hours in advance) and a $135 fee for missed appointments and no-shows. Please let us know via email, phone, text or our contact page if you will not be able to make it to your appointment.

We accept all major credit cards, HSA an FSA accounts. Payment is due at the time of services. For those seeking out-of-network reimbursement, superbills can be provided upon request. ​​
